Effective 1-1-2011, the cost of a one hour Intake appointment is $160. The cost of subsequent fifty minute therapy sessions is $145.

Full payment is required at the time of service except for those clients covered by most Blue Cross Blue Shield plans (I am not covered for government employees), Medicare (I may not be covered by your secondary insurer if you have one) and some Minnesota Care and Medical Assistance plans. For those covered by these plans, it is your responsibility to research what you will owe for a deductible (the amount you pay out of pocket prior to the coverage kicking in) and/or a co-payment so you are prepared to pay these amounts at the time of your appointments. Because I am a provider in your plan, my billing service (KJO Billing, 866-739-8051 or kjold@msn.com) will submit claims on your behalf each month.

You can find a customer service phone number or website address on the back of your health insurance card. The question you want to pose is: “What are my outpatient mental health or behavioral health benefits?”

I may be covered as an “out-of-network” provider for Medica, Health Partners, Cigna, Aetna, United Health Care, and others. Again, you will want to investigate this question with your plan. If you do have such coverage, I will provide you with the documentation necessary to submit a claim directly to your health insurance company for reimbursement directly to you.

For payments at each session, I accept cash, check or credit/debit cards (VISA, Master Card, Discover). Receipts will be provided upon request.

Cancelled appointments are disruptive to my schedule and highly discouraged. If you must cancel an appointment, you are required to give a minimum of 24 hours notice or you will be charged for the appointment. I want to be sensitive to those clients who really need an appointment and would use a session that opens up.
